What is Alpha-Fetoprotein (AFP)?

Alpha-fetoprotein (AFP) is a protein primarily produced by the fetal liver during pregnancy. In adults, elevated AFP levels can indicate certain cancers or liver disease, while in pregnancy, abnormal levels may suggest fetal developmental issues. AFP is measured through a blood test and serves multiple clinical purposes depending on the patient population.

Clinical Applications of AFP Testing

1. Tumor Marker (Oncology)

AFP is most commonly used as a tumor marker for:

  • Hepatocellular carcinoma (HCC): Primary liver cancer; AFP levels often markedly elevated (present in 50-70% of cases)
  • Germ cell tumors: Testicular cancer, ovarian cancer (nonseminomatous tumors)
  • Monitoring treatment response: Decreasing AFP suggests effective therapy; rising levels may indicate recurrence
  • Surveillance: High-risk patients (chronic hepatitis B/C, cirrhosis) often undergo surveillance with ultrasound and AFP, typically every 6 months, though practice varies by guideline and region

2. Prenatal Screening

During pregnancy (typically 15-20 weeks gestation), AFP is measured as part of maternal serum screening for risk assessment, not diagnosis:

  • Elevated AFP: May suggest increased risk for neural tube defects (spina bifida, anencephaly), abdominal wall defects, multiple gestation
  • Low AFP: Associated with increased risk of Down syndrome (trisomy 21) and Edwards syndrome (trisomy 18)
  • Reporting method: Prenatal AFP is commonly reported as multiples of the median (MoM), adjusted for gestational age and maternal factors; cutoffs are lab-specific
  • Follow-up: Abnormal results typically prompt ultrasound examination for further evaluation and may lead to amniocentesis for amniotic fluid AFP measurement and definitive testing
  • Important: Maternal serum AFP is a screening tool that identifies pregnancies at higher risk, not a diagnostic test
💡 Clinical Pearl: Gestational Dating

Incorrect gestational dating is the most common cause of “abnormal” AFP results in pregnancy. Even a one-week error in dating can make AFP appear abnormal because levels fluctuate rapidly in the second trimester. Always confirm gestational age with ultrasound before interpreting results.

3. Liver Disease Assessment

AFP levels can be elevated in chronic liver disease and cirrhosis, even without cancer. Monitoring helps distinguish benign elevation from malignant transformation.

Detailed ultrasound anatomy scan, confirm gestational age with ultrasound, genetic counseling, consider amniocentesis for amniotic fluid AFP if high-risk findings or patient preference.

  • Patient anxiety: AFP results, especially in pregnancy, can cause significant distress. Provide clear education that abnormal results require follow-up testing, not immediate diagnosis.
  • Oncology context: For patients with known or suspected cancer, explain that AFP is one piece of the diagnostic puzzle and will be interpreted alongside imaging and other tests.
  • Sample handling: Use standard venipuncture technique. No special preparation typically required, but verify lab-specific requirements.
  • Trending values: Serial AFP measurements are critical in cancer monitoring. Ensure consistent timing and same-laboratory testing when possible for accurate comparison.
  • Surveillance protocols: High-risk patients with chronic liver disease often undergo AFP testing with ultrasound, typically every 6 months as part of HCC surveillance, though protocols may vary by institution and guideline.

Common Causes of Elevated AFP

Malignant Causes

  • Hepatocellular carcinoma (HCC): Most common malignant cause; 50-70% of HCC cases have elevated AFP
  • Germ cell tumors: Testicular cancer (nonseminomatous), ovarian cancer (yolk sac tumors, embryonal carcinoma)
  • Gastric cancer: Rare AFP-producing gastric adenocarcinoma
  • Metastatic disease: Particularly from hepatic metastases of colon or pancreatic cancer

Benign Causes (Non-Cancer)

  • Chronic hepatitis: Hepatitis B or C infection with active inflammation
  • Cirrhosis: Any cause of cirrhosis can cause mild-moderate AFP elevation
  • Acute liver injury: Drug-induced liver injury, acute viral hepatitis, ischemic hepatitis
  • Liver regeneration: Following liver resection or transplant
  • Pregnancy: Normal physiologic elevation during gestation
  • Hereditary tyrosinemia: Rare metabolic disorder

Causes of Abnormal AFP in Pregnancy

  • Elevated maternal serum AFP: Neural tube defects (spina bifida, anencephaly), abdominal wall defects (gastroschisis, omphalocele), multiple gestation, incorrect gestational dating (most common), fetal demise
  • Low maternal serum AFP: Chromosomal abnormalities (Down syndrome, trisomy 18), incorrect gestational dating, molar pregnancy

Sample Collection & Patient Preparation

Before the Test

  • Fasting: Generally not required for AFP testing, but verify with ordering provider if other tests are being drawn simultaneously
  • Medication review: No medications typically interfere with AFP; continue all prescribed medications unless otherwise instructed
  • Pregnancy dating: For prenatal AFP, accurate gestational age is critical for interpretation. Confirm dating with ultrasound if uncertain—incorrect dating is the most common cause of abnormal results
  • Patient education: Explain the purpose of the test, timeline for results, and that abnormal results require additional testing for confirmation

During Collection

  • Standard venipuncture: Collect blood sample using appropriate tube (typically serum separator tube – SST/gold top)
  • Sample volume: Usually 3-5 mL of blood; verify specific lab requirements
  • Prevent hemolysis: Use gentle technique, avoid vigorous shaking, ensure proper needle size
  • Labeling: Accurately label specimen with patient name, date of birth, date/time of collection. For pregnancy AFP, include gestational age
  • Alternative sample: AFP can also be measured in amniotic fluid during amniocentesis for more definitive prenatal diagnosis when indicated

After Collection

  • Apply pressure: Ensure hemostasis at puncture site, particularly in patients on anticoagulation
  • Transport promptly: Send to laboratory according to facility protocol; serum should be separated within specified timeframe
  • Document: Record collection time, patient tolerance, and any complications
  • Follow-up plan: Inform patient when and how results will be communicated; provide contact information for questions

1. What does a high AFP level mean?
