What Is Heel Pain?

Heel pain is discomfort localized under or behind the heel—often at the plantar fascia origin on the medial calcaneal tuberosity, in the heel pad, or at the Achilles tendon insertion—from overload, trauma, inflammatory or infectious conditions, neuropathy, or vascular compromise. Patients may describe “bruised heel,” stabbing first-step pain, tight Achilles, or burning when neuropathy is suspected.

The heel works with the whole foot and ankle; symptoms overlap with broader foot pain presentations. Plantar symptoms may be associated with plantar fasciitis; posterior heel pain may be associated with Achilles tendinitis among other causes. Hot, erythematous swelling may be associated with cellulitis or crystal arthritis such as gout. Unilateral limb swelling raises DVT consideration per facility algorithms. Common Causes of Heel Pain

Patient Population Differences

Older adults

  • Low-energy falls produce calcaneal and other fractures that younger patients might “walk off”—assume occult injury until evaluated when mechanisms fit

Pediatric patients
