What is uveitis?
Uveitis describes a heterogeneous group of inflammatory disorders in which leukocytes infiltrate the uveal structures. The Standardization of Uveitis Nomenclature (SUN) Working Group framework partitions disease by the primary site of inflammation into anterior, intermediate, posterior and panuveitis, which correlates with symptom clusters, systemic associations and threat to vision. Anterior uveitis (iritis/iridocyclitis) localises to the iris and pars plana anterior to the vitreous base; intermediate uveitis centres on the vitreous; posterior uveitis targets the choroid or retina; panuveitis involves all compartments.

Use SUN terminology in handoffs—mixing “iritis” and “posterior uveitis” loosely obscures risk. Slit-lamp grading of anterior chamber (AC) cells and flare gives objective anchors for response; document the grade the ophthalmologist states rather than inventing numbers if you are not certified to grade.

Clinical presentation
Anterior flares classically produce unilateral deep ache, intense photophobia and circumlimbal injection—pain typically worsens with accommodation because of ciliary spasm. Intermediate uveitis more often presents with painless floaters and slowly progressive vision decline in younger adults, sometimes linked to multiple sclerosis work-up. Posterior segment inflammation may be silent until central involvement causes abrupt reading difficulty or a relative afferent pupillary defect when optic nerve inflammation co-exists.
Who presents atypically?
Children with chronic anterior uveitis tied to juvenile idiopathic arthritis may have white, quiet eyes—screening relies on scheduled slit-lamp surveillance, not symptoms. Older adults or travellers may present with infectious masquerades (syphilis, tuberculosis, toxoplasmosis). Immunosuppressed hosts can develop fulminant necrotising retinitis with minimal external redness; any new floaters plus immunosuppression should be treated as time-critical.
Causes and risk factors
Idiopathic inflammation remains common, but always consider systemic immune disease and infection before anchoring on “autoimmune uveitis”. HLA-B27–positive ankylosing spondylitis spectrum disease is a dominant association for acute recurrent anterior uveitis. Rheumatoid arthritis itself uncommonly causes uveitis but patients may be on steroids or biologics that mask infection. Lupus can produce retinal vasculitis or serous detachments overlapping uveitic patterns.
Infectious triggers include herpetic disease, syphilis, tuberculosis, toxoplasmosis, and endogenous endophthalmitis; shingles with keratouveitis is a classic unilateral painful presentation. Medication-induced uveitis (immune checkpoint inhibitors, rifabutin, some chemotherapeutics) should be reconciled on every admission medication review. Trauma, intraocular surgery and retained lens matter can incite sterile or infectious inflammation.

How is it diagnosed?
Clinical assessment
Visual acuity with pinhole, pupil size and symmetry, basic confrontation fields if trained, and measurement of IOP when authorised are the ward-level bundle before ophthalmology slit-lamp biomicroscopy. Document pain scores and photophobia severity—they track treatment response faster than acuity in some anterior flares.
Laboratory investigations
Ophthalmology and rheumatology select targeted labs: syphilis serology, quantiferon/IGRA or tuberculin testing guided by risk, ACE/lymphocyte subsets only when sarcoidosis is plausible, HIV serology when panuveitis or opportunistic morphology appears. FBC, renal and liver panels support methotrexate or cyclosporine safety.
Imaging
Optical coherence tomography, fundus photography, fluorescein angiography and B-scan ultrasound are specialist-led; nurses facilitate timely transport, dilation counselling and glucose checks when systemic steroids are planned.
Clinical decision flow
- Identify painful photophobic red eye versus painless injection—route urgency.
- Check vision and pupils; new RAPD or severe acuity drop → emergency ophthalmology.

Differential diagnoses
Conjunctivitis shows superficial injection maximal in fornices, little true photophobia, and watery or mucopurulent discharge without fine KP. Acute angle-closure produces corneal steaminess and rock-hard IOP. Keratitis may stain with fluorescein. Multiple sclerosis-associated intermediate uveitis may be painless—do not dismiss floaters in young women with prior neurological events.
Treatment options
Ophthalmologists titrate topical steroids (commonly dexamethasone or prednisolone acetate drops) and long-acting cycloplegics to relieve ciliary spasm and limit synechiae. Periocular or oral prednisone / methylprednisolone pulses are used for severe bilateral or posterior disease. Steroid-sparing immunomodulators include methotrexate, mycophenolate, tacrolimus and biologics such as adalimumab or infliximab for refractory uveitis associated with ankylosing spondylitis or Behçet-like courses per multidisciplinary plans.
Infectious aetiologies receive pathogen-specific antimicrobials; never start prolonged topical steroids alone when infectious panuveitis remains plausible. Macular oedema may require intravitreal or systemic therapy decided solely in specialist settings.
Special populations
Pregnancy limits oral teratogens—only specialist-directed regimens. Children with JIA need prolonged surveillance even when joints are quiet. Older adults may have steroid-induced glaucoma or psoriasis flares when systemic immunosuppression is tapered—communicate skin and joint symptoms to rheumatology.
Clinical practice considerations
Align nursing documentation with SUN class once known. Reconcile every admission against topical steroid drops—patients may use multiple bottles with different potencies. Coordinate dilating drops only per protocol because driving restrictions and narrow-angle risk matter. For tuberculosis-endemic exposure histories, ensure IGRA results are filed before first infliximab infusion where biologics are planned jointly with rheumatology.
Bedside monitoring checklist
- Visual acuity each shift if inpatient; log pain and photophobia scores.
- Check blood glucose on induction doses of systemic steroids if diabetic.

Possible complications
Persistent inflammation causes cataract, band keratopathy (especially in children), cystoid macular oedema, glaucoma from trabeculitis or steroid response, hypotony from ciliary shutdown, and epiretinal membrane formation. Prevention
There is no vaccine for idiopathic uveitis; prevention is secondary—strict adherence to steroid taper, early return for symptom recurrence, control of systemic inflammation in ulcerative colitis and spondyloarthritis, and travel-aware screening for TB before biologics. Eye protection reduces trauma-associated inflammation.
Prognosis & outlook
Isolated HLA-B27 anterior uveitis often relapses but responds to topical therapy with good acuity if IOP monitored. Intermediate uveitis may run a chronic indolent course requiring systemic immunomodulation. Infectious retinitis portends poor outcomes without rapid antiviral or anti-TB therapy—prognosis hinges on time-to-treatment and baseline immune status.

When to seek emergency care
- Sudden severe vision loss, new dense vitritis, or field loss suggesting retinal detachment.
- Neurological deficit, meningismus, or bilateral granulomatous uveitis with systemic illness—central nervous system syphilis must be considered.
