Why does it happen?

Most current guidance and major dermatology societies recommend mycological confirmation (KOH microscopy, fungal culture, PCR or PAS-stained nail clipping) before committing a patient to several months of oral therapy, because nail dystrophy from psoriasis, lichen planus, trauma, melanoma and eczema can mimic onychomycosis and exposes patients to drug interactions, hepatic monitoring and cost without benefit.
How long does oral terbinafine usually need to be taken?
Standard adult dosing for dermatophyte toenail disease is terbinafine 250 mg orally once daily for 12 weeks (6 weeks for fingernails); clinical clearance lags behind mycological cure because the new nail must grow out, often taking 9–12 months for a great toenail to look normal again. Reassess at the end of the course rather than abandoning treatment because the nail still looks abnormal at week 12.
When is itraconazole pulse therapy preferred over terbinafine?
Itraconazole pulse therapy (commonly 200 mg twice daily for one week per month, three to four pulses for toenails) is reasonable when terbinafine is contraindicated or has failed, when non-dermatophyte moulds or Candida are isolated, or when broader-spectrum coverage is needed; the trade-off is a heavier interaction profile (CYP3A4) and the need to screen for cardiac and hepatic contraindications.
Are topical antifungals ever enough on their own?
Topical efinaconazole, tavaborole or ciclopirox can clear mild to moderate distal subungual disease that involves less than about half of the nail plate, spares the matrix and affects only a few nails—particularly when oral therapy is contraindicated. Adherence is the main limitation, since lacquers and solutions are applied daily for 48 weeks with modest complete-cure rates.
What hepatic monitoring is expected with oral antifungals?
Both terbinafine and itraconazole carry hepatotoxicity warnings; baseline liver function tests are recommended and many services repeat them around 4–6 weeks into treatment, with earlier review if jaundice, dark urine, anorexia, nausea or right-upper-quadrant pain develop. Coordinate with the prescriber on local pathways and stop the drug for unexplained transaminase elevation.
How should diabetes change the management plan?
Patients with diabetes have higher onychomycosis prevalence and greater risk of cellulitis, ulceration and limb-threatening infection from fissures and subungual portals. Lower the threshold for prompt treatment, coordinate with podiatry for safe debridement, screen and treat concurrent tinea pedis, and escalate the same shift if erythema, warmth, pain out of proportion, lymphangitic streaking, fever or new ulceration appear.
Why do fungal nails recur after apparently successful treatment?
Recurrence rates of around 20–25% within a few years are well described, driven by untreated tinea pedis reservoir, contaminated footwear and shared shower spaces, biomechanical nail trauma, immune or vascular comorbidity, and incomplete eradication from the nail matrix. Sustained foot-care, antifungal powder in shoes and treatment of any active interdigital tinea reduce relapse.
What features should make a clinician question the diagnosis?
Single-nail involvement (especially a thumb or great toe) with longitudinal pigmentation, rapid growth, a Hutchinson-like nail-fold pigmented streak, marked nail-fold inflammation, pitting, severe matrix destruction with overlying skin disease, or persistent abnormality despite negative mycology should prompt re-evaluation for nail psoriasis, lichen planus, trauma, chronic paronychia or subungual melanoma—biopsy if uncertain.
Are oral antifungals safe in pregnancy and breastfeeding?
Routine onychomycosis is not life-threatening, so most authorities recommend deferring oral terbinafine and itraconazole during pregnancy and lactation; itraconazole in particular is contraindicated in pregnancy. Limited topical therapy or simple debridement is preferred until after the perinatal period unless a stronger indication exists.
