How is it diagnosed?
Examination & imaging
Firm, irregular, non-transilluminating intra-testicular masses prompt urgent ultrasound. Extra-testicular lesions more often prove benign, whereas intra-testicular hypoechoic vascularised solid tissue tilts toward malignancy—still, the definitive step is surgical pathology after inguinal orchidectomy.
Laboratory investigations

Staging imaging

Differential diagnoses

Metastases to the testis from prostate carcinoma, lung primaries or lymphoma exist but are rarer; persistent solid intra-testicular architecture should bias toward orchidectomy rather than endless antibiotic trials.
Treatment options
Surgery
Radical inguinal orchidectomy remains diagnostic and therapeutic. Partial orchidectomy is exceptional and MDT-governed. Retroperitoneal lymph-node dissection is staging or therapeutic in selected NSGCT pathways and carries ejaculatory/autonomic morbidity—pre-op fertility discussion is mandatory when nerve-sparing intent exists.
Medical oncology
Cisplatin-based regimens anchor metastatic curative intent; etoposide replaces older vinblastine in BEP. High bleomycin cumulative exposure amplifies lung fibrosis risk—nurses should daily query cough, dyspnoea, and incentive spirometry adherence when used. Antiemetics align with local emetogenicity policy—commonly combining 5-HT3 antagonists such as ondansetron with NK1 pathways; add metoclopramide or others only per charted prescription.
Radiotherapy
Adjuvant para-aortic radiation is an option for some stage I seminomas; teach gastrointestinal and fatigue expectations and emphasise protection/shielding conversation for the contralateral testis when relevant.
Surveillance
Stage I pathways lean on schedules combining physical exam, markers and interval imaging—non-adherence drives relapse detection delay; give patients written timelines and psychology referrals when scan anxiety paralyses attendance.

Possible complications
Treatment-related neuropathy, ototoxicity, pulmonary fibrosis, cardiovascular disease, metabolic syndrome, anxiety/depression, infertility, and therapy-related acute leukaemia (especially with high cumulative etoposide) are all recognised—documentation should reflect structured toxicity grading so adjustments happen before irreversible organ injury.
Prevention
There is no population screening programme akin to mammography; clinician-facing prevention is early referral of persistent testicular abnormalities, correction of undescended testes in childhood where appropriate, and genetic counselling when syndromic cues appear. Teach motivated high-risk survivors monthly self-examination of the remaining testis as adjunct—not replacement—for oncology follow-up.
Prognosis and outlook
Overall survival is among the best of solid tumours in young men because platinum cures even advanced IGCCCG good-risk NSGCT in most cases; poor-risk biology still achieves long-term cure in a meaningful subset but needs intensive supportive care. Relapse after initial chemotherapy warrants tertiary subspecialty planning (high-dose strategies, trial enrolment) rather than ad hoc single-agent guesses.

Pre-treatment
Verify pregnancy testing policy for partners when relevant, contraception advice (defer conception ~3 months post cisplatin per guideline summaries), hepatitis/HIV screening per unit, and provide translated education leaflets where English is second language.
During treatment
Use central venous access bundles when PICC or port devices are in situ; protect hearing baseline when services offer audiometry; encourage chewing gum for cisplatin-related taste disturbance when tolerated.
