Corrected Calcium Calculator

Adjust total calcium for albumin as a screening estimate. Do not use corrected calcium as a substitute for ionized calcium when clinical decisions depend on true calcium status.

Understanding this tool

Why nurses use it

Total serum calcium falls when albumin is low, but ionized calcium may still be normal. Corrected calcium gives a screening estimate when albumin-adjusted interpretation is used locally — though ionized calcium is preferred for critical decisions.

When to use it

Use when total calcium is abnormal and albumin is low (e.g. critical illness, liver disease, malnutrition). Do not rely on corrected calcium alone when treating symptomatic hypo- or hypercalcaemia.

How the calculation works

Corrected Ca = Total Ca + 0.8 × (4.0 − albumin). The factor 0.8 reflects how much calcium binds to albumin.

Worked example

Ca 8.0 mg/dL, albumin 2.5: 8.0 + 0.8×(4.0−2.5) = 9.2 mg/dL (≈ 2.30 mmol/L) corrected.

Limitations & clinical notes

  • Corrected calcium correlates poorly with ionized calcium in critical illness.
  • Do not substitute for ionized calcium when treating symptomatic hypo/hypercalcemia.
  • Adult estimate — ionized calcium is preferred for pediatric and critical care decisions.
