Understanding this tool

Why nurses use it

NEWS2 is the standard adult early warning score in many UK and international hospitals. It helps nurses recognise deterioration early and triggers escalation when physiology deviates from normal.

When to use it

Use for routine adult vital sign sets per local policy — especially ward, AMU, and emergency settings. Select SpO₂ Scale 2 only when hypercapnic respiratory failure is documented and the prescribed target is 88–92%; otherwise use Scale 1.

How to use this calculator

  1. Select SpO₂ Scale 1 (standard) or Scale 2 (documented hypercapnic respiratory failure with 88–92% target).
  2. Enter each physiological parameter from the latest observations.
  3. Indicate if the patient is on supplemental oxygen.
  4. Sum the score and apply your local escalation threshold.
  5. Re-score with each observation round or when the patient deteriorates.

How the calculation works

Each parameter (RR, SpO₂, BP, pulse, temperature, consciousness, oxygen) maps to points on the RCP NEWS2 chart. Scale 1 scores low saturations for most patients; Scale 2 uses an 88–92% target range and flags high saturations on supplemental oxygen. The total score reflects illness severity and triggers graded responses.

Limitations & clinical notes

  • SpO₂ Scale 2 is only for documented hypercapnic respiratory failure with a prescribed 88–92% target — use Scale 1 for all other patients.
  • The decision to use Scale 2 must be made and recorded by a competent clinician per RCP NEWS2 guidance.
  • Score triggers vary by hospital escalation policy.
