Osmolal gap (measured minus calculated osmolality) suggests unmeasured osmoles — toxic alcohols, ethanol, or other substances in ingestion workups.

How the calculation works

Calculated osm ≈ 2×Na + glucose/18 + BUN/2.8 (US units). Gap = measured − calculated.

Osm gap = Measured osm − Calculated osm (2×Na + glucose/18 + BUN/2.8)

Worked example

Measured 310, calculated ~290 → gap ≈ 20 mOsm/kg (investigate unmeasured osmoles).
