Positioning ladder and sternal-angle reference
Start with the patient supine with hips flexed so the trunk can elevate as one plane. Raise the head of bed in steps—many teams use 30–45° first, then adjust until the venous meniscus is visible or the patient’s symptoms limit further elevation. Record the angle every time.
The angle of Louis (sternal angle) is the usual zero reference for vertical height. Some protocols add a fixed distance from that angle to estimate right atrial pressure; the correction factor varies with position. Unless your policy assigns nurses a numeric formula, describe visible height relative to the sternal angle and clavicle.
Waveform cues nurses may notice
Full waveform interpretation is physician scope, but recognising double flicker versus a single carotid thump helps you document accurately and prompts timely medical review when patterns change.

JVP Assessment Procedure Steps
Hand hygiene, identity, and consent
Perform hand hygiene, verify patient identity, explain you will look at neck veins while they rest semi-upright, and ensure privacy. Ask about neck pain, recent surgery, or dizziness when sitting up.
Position the patient
Elevate head of bed to institutional target (often 30–45°). Turn head slightly away from your examining side (usually leftward when standing on the patient’s right). Keep shoulders supported; loosen tight collars.
Locate the right internal jugular vein
Identify the triangular space between the sternal and clavicular heads of the sternocleidomastoid. Use tangential lighting. If the right IJV is not seen, inspect the left IJV and document which side was used.
Confirm vein versus carotid
Palpate the radial pulse while watching the neck: carotid upstroke is single and palpable. The IJV should show double flicker, fall with inspiration in most patients, and not feel like a firm arterial tap.
Estimate venous column height
Identify the highest stable point of pulsation. Measure vertical distance above the sternal angle if your skill mix includes measurement; otherwise describe relation to clavicle and angle of Louis. If veins are only visible when upright, note that elevation suggests high central venous pressure.
Correlate with lungs, periphery, and vitals
