What Dependent Edema Means
Dependent edema is tissue swelling that accumulates in the lowest body regions when gravity pools fluid—classically the feet and ankles after standing, or the sacrum and flanks when supine. Patients may say their shoes feel tight, socks leave deep lines, or their legs feel “heavy” by evening. It often overlaps with broader leg swelling when the process involves calves and thighs, not only the distal segment.
The pattern is not a diagnosis. It may be associated with venous hypertension, right- or biventricular fluid overload, renal sodium retention, hepatic synthetic or circulatory dysfunction, medication effects, lymphatic impairment, or localized inflammation and thrombosis—among other possibilities. Nurses describe distribution, timing (worse at day’s end vs constant), response to elevation, and associated edema elsewhere; clinicians integrate labs, imaging, and history.
“Dependent” emphasizes where fluid collects, not why. A patient with bilateral evening ankle puffiness and clear lungs may still need a different workup than one with the same appearance plus new pleuritic pain and tachycardia. Trend and context beat a single snapshot.
Common Causes of Dependent Edema
