What Are Palpitations?
Palpitations describe an uncomfortable awareness of the heartbeat—pounding, fluttering, racing, flip-flopping, or a brief “pause” or skipped beat. Some patients localize the sensation to the chest; others feel neck pulsations or vague thoracic unease. Intensity ranges from brief annoyance to sustained distress.
Palpitations are a symptom, not a diagnosis. They may be associated with premature atrial or ventricular beats, atrial fibrillation, other arrhythmias, electrolyte disturbance, thyrotoxic states, medications, stimulants, anemia, or structural heart disease—and sometimes with heightened awareness of a normal rhythm. Compare with irregular heartbeat when the main finding is pulse irregularity rather than subjective pounding alone.
A patient may report severe palpitations while the monitor shows sinus rhythm, or minimal symptoms during nonsustained arrhythmia. Pair the story with rate, blood pressure, associated features, and captured strips when available—avoid reassuring from a single normal ECG alone.
Common Causes of Palpitations

- Benign or situational awareness: Anxiety, pain, fever, anemia, pregnancy, caffeine, decongestants, or nicotine may increase sympathetic tone or awareness of a normal rhythm—still document new or escalating symptoms.
- Atrial fibrillation or flutter: Rapid or irregular rates may be associated with hypertension, valvular disease, thyrotoxic states, alcohol (“holiday heart”), obesity, or post-operative stress—rate control and anticoagulation decisions are prescriber-led.
- Supraventricular tachycardias: Sudden onset–offset racing heart may be associated with AVNRT, AVRT, or atrial tachycardia—document onset, vagal maneuvers only per protocol, and hemodynamic status.
